What information is needed from me when I call to schedule my first appointment?

• Your name.
• If you are not the patient, the patient’s name.
• Patient’s date of birth.
• Patient’s social security number (for interoffice    identification only.)
• Phone number.
• Address.
• Your insurance carrier.
• Subscriber’s name.
• Subscriber’s employer.
• Subscriber’s social security number.
• Subscriber’s date of birth.
• Subscriber’s insurance identification/member number.
• Group number, if applicable.
• Insurance carrier’s telephone number.
• Do you have secondary insurance coverage?
• If so, the above information pertaining to the other    coverage.
• How were you referred to us or how did you hear    about our services?
• What are your major issues of concern? Why are you    seeking our services?

How will I know whether your services are covered by my health insurance?

In most cases our office manager will be aware if our services would be covered or not. After taking your insurance information our office manager will contact your insurance carrier to verify your eligibility and benefit information. This phone call is for information purposes only. It is not a guarantee a benefits. Benefits will be determined once the claim has been submitted. It is your responsibility to follow through with any contacts that must be made to an employee assistance program, if applicable. Failure to do so could result in reduced or no benefit.

 

How long will the initial evaluation take?

The initial evaluation with a therapist is scheduled for 90 minutes.The initial evaluation with a psychiatrist is scheduled for 45 to 60 minutes.

How long are the follow-up sessions?

Follow-up sessions with a therapist are scheduled for 60 minutes.Follow-up sessions with a psychiatrist are scheduled for 15 minutes.

 

What, if anything, do I need to do to prepare for my first meeting?

After your first appointment is scheduled a packet of paperwork and information will be mailed to you. It is important that this information be completed prior to your appointment and brought with you or returned by mail at least one week prior.

You could also have your medical records from another office, physician, or service transferred to this office if that information would be helpful to the service you’re seeking from our office.

Sometimes it may also be helpful to simply write down your agenda, your expectations and any questions for this meeting. That way something that may be important for you to address will not be overlooked or forgotten.

Where will I wait before my appointment?

We are the only office in our building. Our driveway is a private driveway (not shared with other businesses.) You will enter into our waiting area. During normal business hours you may check in with the receptionist. During some evening hours and Saturday hours you would merely make yourself comfortable in the waiting area. Our therapists have their schedules and know when someone will be waiting for them.

As parents, do we get to sit in on our child's session?

Especially in the situation of a first meeting a parent or parents will be asked to join in. We will need some background information and the parent’s perspective of what is happening and an understanding of their concerns. At some point the parent(s) may be asked to step out of the meeting.

What are my privacy rights?

Confidentiality and privacy issues are very important to our staff, your right and conducive to developing a rapport between patient and therapist. We cannot give information to another person without your written consent to do so. We cannot confirm or deny that a person is a patient of this office without their written consent to do so.

 

When the patient is a child and the parents are divorced we will only confirm appointments, change appointments or discuss any information with one parent unless there is written consent to share with the other parent. In the case that a court order is involved regarding medical records and information, please remember that a court order is only an agreement between the parents and the court. Our office is not a party to the order. Parents must communicate with each other, if not for correct and current information, but for their child’s best interest and welfare.

How do I know which therapist to see?

Through your contact with our office manager you and she will determine which therapist may be chosen to meet with you. Matching a patient to a therapist is done by therapist’s area of specialty, age of patient the therapist works with and availability. All of our therapists are licensed clinical social workers and can work with persons dealing with most mental health issues. Some of our therapists are also certified alcohol and drug counselors and would be able to work with persons dealing with those issues. With respect to the age of a patient, not all of our therapist work with children.
